New online Leave of Absence request form for SGPS students
On Monday, July 15, the School of Graduate and Postdoctoral Studies (SGPS) will launch a new online Leave of Absence (LOA) request form for graduate students who wish to take a leave of absence from their studies.
Students will access the form through the Self Service option on . LOA requests for Fall 2019 and onward must be completed through MyCampus; paper forms will no longer be accepted.
Note: All approvals will be completed online. Faculty members must have network access to review an LOA request. If a graduate student you supervise requests an LOA, you will receive an email directing you to the approval workflow.
Leaves of absence are governed by the Graduate Academic Calendar regulations, including eligibility and duration. For more information, visit the section of the Graduate Academic Calendar.
